Affidavit of support and guarantee — what immigration officers look for, and a sample to fill in

Ready, not blank.

Fill it in and download it in Word for free.

Short answer. The guidelines call it an Affidavit of Support and Undertaking. A sponsor abroad has it authenticated by the Philippine Embassy or Consulate, and it shows the relationship within the fourth civil degree, the sponsor's financial capacity and legal status, and contact details. A local sponsor traveling with the passenger uses a notarized affidavit. It does not guarantee departure.

Questions

Can a friend sponsor me?

The guidelines have a separate item for a sponsor who is another person or a company: the affidavit is authenticated by the Philippine Embassy or Consulate.

Does it guarantee that I can leave?

No. The Immigration Officer decides.
