Last day to file: type the date of discharge
File the motion by: type the date
15 days from receipt — PhilHealth Circular No. 03, s. 2008. Check with PhilHealth that this period still applies; follow the period in your notice.
Rule: PhilHealth Circular No. 20, s. 2014 — 60 days, 180 abroad. Check your own case.
Calculated by the rule of PhilHealth Circular No. 20, s. 2014: 60 calendar days after discharge, 180 if abroad, counting from the day after discharge. Check your own case — calamity extensions and package rules differ.

Deadlines
| What | Deadline | Source |
|---|---|---|
| Claim filed directly by the member | 60 calendar days after the date of discharge | PhilHealth Circular No. 20, s. 2014, Part III, item 7 |
| Confinement abroad | 180 days after the date of discharge | PhilHealth Circular No. 20, s. 2014, Part III, item 9 |
| Emergency in a non-accredited hospital | 60 days after the date of discharge | PhilHealth Circular No. 20, s. 2014, Part III, item 8 |
| Claims filed by the hospital | 60 calendar days — the hospital’s duty | e.g. PhilHealth Circular No. 2026-0008, item 7 |
| Motion for reconsideration of a denied claim | 15 days after you receive the denial — check that this still applies | PhilHealth Circular No. 03, s. 2008 (status not confirmed) |
Count calendar days starting the day after discharge. Areas hit by a calamity may get longer periods by a separate PhilHealth circular. Direct filing is allowed only in some cases — for some newer packages only for confinement abroad.
Checklist
Step by step
1
Ask the billing office to deduct PhilHealth from the bill. Most claims are filed by the hospital.
2
Abroad, emergency in a non-accredited hospital, weekend or holiday confinement without the employer’s certification, and a few other cases.
3
Type the discharge date above. File well before the last day.
4
CF1 by you; CF2, CF3/CF4 and the waiver by the hospital; SOA and original receipts.
5
The date stamp proves you filed on time. If denied, file the motion for reconsideration.

60 calendar days after the date of discharge; 180 days if you were confined abroad (PhilHealth Circular No. 20, s. 2014). A claim filed late is denied, and PhilHealth has said late filing is the main reason for denied claims.
Claim Form 1, Claim Form 2, CF3 or CF4 (or a clinical abstract), the hospital’s waiver for directly filed claims, the itemized statement of account, and original official receipts — plus extra papers for surgery, confinement abroad or a representative. See the checklist above and confirm with your PhilHealth office.
Only in some cases: confinement abroad, an emergency in a hospital not accredited by PhilHealth, an employed member confined over a weekend or holiday who could not get the employer’s certification, and other cases PhilHealth lists. Otherwise the hospital files the claim and deducts PhilHealth from your bill.
A directly filed claim is paid at the case rate for your condition or procedure, not the whole bill.
You may file a motion for reconsideration with the PhilHealth office that denied it. The template above lists the usual grounds. A 15-day period from receipt is commonly cited (Circular No. 03, s. 2008) — confirm with PhilHealth and follow any period written in your notice.
